Grievance Process & Submission Form

We believe all individuals have the right to be treated with dignity and respect.

Anyone can file a complaint with the OMH/CSA about a behavioral health provider. A complaint could include a problem or concern regarding quality of care, staffing or unsafe conditions. The OMH/CSA will work to resolve the complaint and address the concerns with the provider.

To file a complaint, fill out the Provider Complaint Form and send it to Jmiller@harfordmentalhealth.org.

If you need assistance completing the online form, please call our office at 410-803-8726 (press 1 for adult services or press 5 for child and adolescent services).
